Scheduling an appointment is simple.
Give us a call during office hours. We make our best efforts to schedule a time that’s most convenient for you. We strive to see all fracture referrals within 24 hours. Remember that Physiatry and Neurology appointments require a Primary Care Referral. Early morning appointments are also available.

When you call, we will ask for information about the reason for your visit. That way we can schedule enough time for you to meet with the appropriate specialist.
We ask that you arrive 15 minutes before your scheduled appointment. This gives you time to register and read through all the necessary paperwork. To make things go as smoothly as possible, have your photo ID and insurance card ready. We will also collect your co-pay at the time of the visit.
Prior to your visit, please download our new patient form, fill it out, and bring it with you to the appointment.
